Abstract:
In an effort to learn more about statewide multi-stakeholder collaborations that have been successful in improving measurable clinical outcomes, the following three collaboratives were chosen in July 2002 for review as potential models for California: 1. Minnesota's Institute for Clinical Systems Improvement (ICSI); 2. The Washington State Diabetes Collaborative (WSDC); and 3. The California Cooperative Healthcare Reporting Initiative (CCHRI) Diabetes Continuous Quality Improvement (CQI) Project. The term "structured collaborative" refers to a specific structure and process to support provider organizations with interventions to improve the quality of their clinical care. A structured collaborative targets provider organizations for two reasons: first, because they are the entities that actually provide the care; and, second, because they are systems of care delivery, as opposed to individuals providing care.
